Educate, Train and Improve Quality Care
The Oregon GWEP’s purpose is to help the health care, public health, and supportive care teams of Oregon learn about providing age-friendly and dementia-friendly care for older adults. We are partnership-focused and value collaboration across the state of Oregon. Our goal is to reach ALL Oregon communities to provide education and training. We will engage patients and families to address care gaps and improve health outcomes that are important to Oregonians.
We have three objectives:

Develop Reciprocal Partnerships
Develop strong, collaborative partnerships between teaching hospitals, primary care sites (including nursing homes), and community organizations, that transform clinical training environments into age-friendly and dementia-friendly care sites.

Provide Clinical Training
Provide interprofessional geriatrics clinical training and education to students, residents, fellows, faculty, and preceptors in Tribal, Tribal Organizations, Underserved and/or Rural (TTOUR) clinical care sites. We want to work with training programs to increase the number of trainees who practice in TTOUR sites following graduation.

Support Ongoing Education
Establish and maintain education and training programs in primary care sites that provide the supportive care workforce, direct care workers, and primary care workforce with the knowledge and skills to improve the care of older adults, including persons living with dementia. We also support ongoing age-friendly education for patients and families.
Our Team
We are an interprofessional team that is passionate about improving care for older adults across Oregon.
